For new patient appointments, please bring the completed health history and patient registration form to your first appointment.
We require previous medical records or a referral to schedule the first visit with any of our Endocrinologists. Given a recent physician retirement, we are only scheduling new appointment for thyroid cancer, hyperthyroidism, Type 1 diabetes (juvenile), hyperparathyroidism (hypercalcemia), adrenal and pituitary disorders. Please fax medical records or have a referral sent to the office location desired to schedule a new patient appointment.
